Dynamic values lists not populated

I have used this post http://blogs.msdn.com/b/crm/archive/2008/02/19/e-mail-to-case-lead-using-crm-4-workflow.aspx to build a workflow that creates a case from a received email, change the status of the original email and send a confirmation email to the sender.

All seems to be working until you look at the case number referenced on the original email and the confirmation email. They aren't the case number assigned when the case was created.

On the Create Case screen I don't see the field Autocreated Case either already in the Title field or in the Dynamic Values for me to choose. Although once the case is created it does have an case number generated by autonumbering.

And I assume that that's why I don't get that case number displayed when I reference {Case(Create a Case(Case))} in the original and confirmation email.

Also, once I choose a value from the drop-down list in the forms assistant I am unable to choose any other values associated with an email. To clarify, when I have a blank Create Case screen and I choose Email from the Look In list I can then choose all /any email field but once I have chosen the first email field to add all of the remaining email fields disappear. It makes no difference if I move to another field on the Create Case screen.

I apologize for being so long-winded but I am somewhat frustrated with this and have tried, a number of times to correct it.

Many, many thanks for any direction or assistance you can give me. If I've missed something in the documentation or overlooked a previous post I apologize and would appreciate being pointed in the right direction.

My current configuration is Dynamics CRM On premises (Update Rollup 13) eMail Router (Update Rollup 13)
