How t Load /Show MSCRM Lookup Field values in Custom HTML Web Resource Pick List items???

Hello ,

    I have a requirement in which i have a Custom HTML Page ,i want to load  the custom entity  records from my CRM  into the  Pick List filed  present in the Custom HTML .Want to know how to store the GUID and Name of the records in my Pick List Field
