why custom fields do not appear on form automatically ?

I m adding an entity but its strange custom fields I add do not come on form automatically. I have to go form and manually put field on form. Same happen if I add a field in existing entity. 

I always press save button and publish but not sure what I m missing. Appreciate your help.
