create a process that creates a or multiple new annotation IDs in Accounts

Hey!

I'm trying to write a process that will create a "test" note inside of Accounts entity. The purpose for that is to allow me to export notes from opportunity and import them to their corresponding Accounts. 

By creating notes inside of accounts, I would be able to query/export all notes whether in Opportunity or in Accounts and then copy the opportunity notes in their corresponding accounts thereby deleting the newly created account notes. Re-importing that excel file into the CRM would update the account notes with their corresponding opportunity notes. 

So far I've created an ACTION Process from the NOTES entity which checks if an Account is active then "Create Record" inside of notes with a test value in the titles and body of the note. No results.

I've also created a WORKFLOW process "as on demand" with the same parameters as the previous action.
