Copy email and phone number of contact to custom fields on Pipeline form

We have a form called Pipeline.  It is tied to a CRM Account (Company).  On the form we have a contact tied to the Main Account (Company).  Some accounts have multiple contacts, so we have them select the contact related to the Pipeline or Opportunity.  I would like to auto fill a custom email field and custom phone number field on my pipeline form after selecting the contact using a java script.  If the contact email and phone fields are blank then leave the custom fields blank for the user to fill in.   We will use the custom fields in an email that will be sent to select users that may not have CRM access to identify how to get a hold of the contact.  I can pull the CRM ID but cannot get the email or phone number.  We are on hosted CRM 2011.
