Voice of the Customer - Enter responses from offline survey

We need to be able to enter responses into voice of the customer tied to a specific customer that has filled out an offline paper survey.  i.e. they fill out a survey on paper and we then enter their scores and responses for them into Voice of the Customer so it can be tied to their record in CRM.  Is this possible?
