BCM to CRM data migration

Is there a tool to migrate all data from the discontinued BCM Business Contact Manager for Outlook 2013 to Microsoft Dynamics CRM?

We want to keep custom fields created in BCM and all history in the Microsoft Dynamics CRM Database.

Is that possible?
