how to debug the Js in below scenario

I have a custom entity called "Phone Call". In contact form there is custom button "Create Phone Call" on click of the button "Phone Call" form should get open, while form is loading script error is coming. But when we open the same form seperately that script error is not comming. How to Troubleshoot/Debug this?

Can some one help me with clear explaination.
