Insights by Insideview -- how to setup in custom sitemap? dynamics 365

I removed the insights tab from CRM (and moved the "Discovery Center" shortcut to another place) so when I updated to Insights to 4.2, it did not update my sitemap icon/shortcut and now the shortcut does not load.

Currently I get an error saying HTTP Error 404 - Not Found when opening up the discovery center shortcut.

Here is my current URL for the shortcut that was setup when I had 4.0 installed.:

iv_/webpages/discovery_center.htm?data=solutionVersion=4.0&crm_version=v125

I tried changing it to 4.2 and that did not help.
