Description layout - Dynamics 365

Hi guys,

I have a weird issue about the description field in case entity. When I just click on this field, the case record is updated (I didn't did any change, just clicked on this field). I checked the audit summary view and I found that I "updated" this field. What I found, is the same content but with a new layout:

Why I have this new layout? and how can I avoid it?
