I am working on a client project. I should have rolled this out into production but one particular issue has been preventing me from deploying it. I have been searching for a possible solution but always fail to find one. This project has been developed using CRM 365(Online) v9.0. The issue is that any or each stage would appear more than once in the Notes section of my main form after being completed.

Below is the workflow which runs in the background to creates the stage duration record:
Sometimes, the three stages are captured perfectly, yet sometimes each stage appears twice or thrice.
Please help me resolve this issue.
