CRM version 9 - Ribbon Workbench Subgrid + click no longer works

Upgrade CRM to version 9 onPremise.  I use the Ribbon Workbench pretty heavily in our application.

We have 3 different case forms.    On our account screen we have 3 different case subgrids.   Depending on which + button the user clicks it opens the appropriate case form.

This is no longer working in version 9 of CRM.  Not sure why.  The code is still there within the Ribbon Workbench.  Here is an example:

It doesn't appear that the webresource and javascript function I am pointing to gets fired at all.   I just put a simple console log in there and nothing.

Any help is appreciated.
