Show Modal Dialog with Preview box on CRM dynamics

Is there any possible way we can achieve the insert template look and feel on another entity ribbon workbench. 
I want to add a button on case entity form, which would open up to copy some template and then use the template.

Is it possible to do that with customizations in CRM?
