Dynamic CRM Appointment Description Format Changed after 15 mins

Hi,

The Description Field under the Appointment Activity changed its format after 15mins if the appointment is not marked complete after 15mins.

Eg

Original Format

Name:

Address:

Contact:

After 15mins the format changed to below.

Name:Address:Contact:

Does anyone have any idea why and how to resolve this?
